Each program is designed around a specific barrier people run into: the cost of care, an empty pantry, a prescription that cannot wait. All of them are free to the people we serve.

What each program is built to do

We describe what these programs are designed for rather than claiming results we have not measured.

  • Community Health Access Program

    Designed to lower the cost and confusion of getting medical care.

    This program helps connect people who are uninsured or underinsured with providers, clinics, and assistance options they may not know exist. Support can include help understanding a bill, finding a provider, or applying for available financial assistance.

    For adults and families facing medical costs they cannot cover

  • Healthy Families Initiative

    Focused on the everyday essentials that keep a household healthy.

    Food support, nutrition education, and preventive health guidance are offered together, because a family's health rarely depends on any one of them alone. The goal is steady, practical help through a difficult stretch.

    For households working through food and health insecurity

  • Homeless Outreach

    Designed to meet people where they are, without preconditions.

    Outreach focuses on immediate needs such as food, hygiene, and a route to medical attention, and on helping connect people with housing and social services. Every conversation starts with dignity and respect.

    For neighbors experiencing homelessness or housing instability

  • Youth Wellness Program

    Focused on health habits that start early and last.

    Age-appropriate education, wellness activities, and referrals help young people and their caregivers understand and act on their health. The program is built to complement, not replace, a child's regular care.

    For children, teens, and the adults who care for them

  • Senior Care Assistance

    Designed to help older adults stay well and stay connected at home.

    Assistance can include help with medication access, guidance through healthcare paperwork, and referrals to community services that reduce isolation. Support is paced to the person, not the process.

    For older adults living independently or with family

  • Emergency Relief Fund

    Set aside for the moments a household cannot plan for.

    The fund is directed toward urgent, short-term needs such as a prescription, a utility shutoff, or transportation to a medical appointment. Requests are reviewed individually and confidentially.

    For individuals and families facing a sudden crisis
